If you are in treatment with a primary care doctor or other medical professional, or if there will be anyone else that you want me to contact about your treatment, please download and complete a release form.

Release of Information

If you are insured through United Behavioral Healthcare, then you can complete one of their Initial Wellness Assessments. I will go over the results with you at the first visit and then fax the form to UBH.
